This short form is for you, the Event Host, and is the next step after completing your booking and arrival date. After you submit this form, you will be able to access our Event Guest Form and forward it to all of your guests so we can collect dietary restrictions, personal preferences, and emergency contact information. The form also includes the mandatory indemnification waiver that must be signed by all guests prior to arrival. If you plan to attend the event, we need to you to complete this second form as well.
